Under the supervision of the Executive Director, the Controller is responsible for maintenance of the general accounting records. Will prepare or supervise the preparation of the appropriate financial reports, and coordinates information systems functions. In addition, this position will be responsible for all aspects of financial management, coordinates and oversight of tax returns and financial reports to local, state and federal funding agencies and or taxing government agency. The Manager of Quality and Population Health (Director) is responsible for the management of the medical clinic operations of SBHF, as well as providing a broad range of clinical and administrative assistance to patients. This role, along with the President & CEO, Physicians, Director of Operations, Registered Dietitians, Health Educators, Supervisors, and supporting staff help support the Patient Centered Health Home model of care. The Manager of Quality and Population Health helps plan and coordinate the development and implementation of San Benito Health Foundation (SBHF) population health initiatives, including, but not limited to: UDS, Clinical Measure, HEDIS, SBHF Quality Measures, and other program quality measures. This position will also mentor Patient Care Specialists and other staff.
This role serves as the leader of strategic programs designed to improve clinical outcomes through coordination of clinical operations, information systems, administrative departments, physician leadership, and physician practices. Determines direction, goals, and objectives of assigned programs and links to appropriate hospitals, physician.
Duties and Responsibilities:
- Accountable for the management and oversight of the population health management of the clinic.
- Provides reports as required by Federal, state and local laws and regulations
- Presents updates on special project to the Board of Directors
- Serves as the coordinator of strategic programs designed to improve clinical outcomes through coordination of clinical information systems, administrative departments, provider leaders and provider practice.

The Registered Dietitian serves as a case manager to our patients with chronic conditions. Within the WIC department, the Registered Dietitian oversees the nutritional component of the program. The position requires extensive experience working in a community health setting providing health education to low-income minorities in a multi-cultural setting. The Health Educator is responsible for the provision of health and wellness education and advocacy for health care and facilitation of access into the health care system for members of the community served by SBHF. Other duties: Evaluates, designs, presents, recommends and disseminates high quality, culturally appropriate health education information / materials; Coordinates a wide range of health education services, including establishing effective referral and publicity systems, monitoring quality, and facilitating use of the community services to promote the delivery of cost-effective health education. Educator will assist in implementing the Rural Outreach Project to members of the isolated rural community.
The Health Educator must possess a BA/BS in health education, public health, nutrition or a related field. Experience in health and wellness field preferred. Strong interpersonal skills and the ability to work in a team environment. Possess a thorough knowledge and understanding of health behavior change theories and their application. A working knowledge of motivational strategies, materials development, and training is required. Bilingual in Spanish/English is required.
